are you editing local.cf or sa-mimedefang.cf? Mimedefang does not use local.cf. Since your FBSD, you might be using the port of mimedefang, it puts its spamassassin config in /usr/local/etc/mimedefang/spamassassin/sa-mimedefang.cf
Actually, MIMEDefang *does* use local.cf. It just tries sa-mimedefang.cf first.
The simplest thing to do (if you want the same settings in command-line SA and SA called by MD) is remove sa-mimedefang.cf and just put everything in local.cf.
