We transparently learn any outgoing or incoming mail as spam or ham
respectivelly.
If it scores over the threshold it's learned, and rejected if spammy
irrespective of whether it's going in or out.
We use MD+SA-2.63+surbl,
+pyzor+dcc,
+razor,
+evilrules.cf,
+drugs.cf.
Sare rules, and surbl are really really good.
