I personaly don't need to maintain much, it takes me a few minutes per week in order to execute a "learn" script which runs sa-learn on all spams received during the week and to erase them afterwords. I also had to create some custom rules at some point, in order to target some specific spam, but that took about 10 minutes. I dont see any real maintenance in SA, just make sure you start with a large spam database so your Bayes is accurate, and try some of the custom rules released by SARE. thats about it.. few minutes per week or month depending on how much spam you get.
