We use sa-exim for greylisting. It allows thresholds for when to greylist. For example, we greylist email that score between 3.0 and 10.0. That way if an email is ham it will notice no delay in making it to the recipient and if it is very much spam it will just be dropped without wasting resources.
We have had very dramatic drops in spam getting through by greylisting. I'd say about 90% of email scored between 3 and 10 is never resent. I believe MD has very similar abilities to sa-exim.
