If you right-click on blank space anywhere in the Outlook toolbars, you'll
get a toolbar selection menu pop-up. Choose "Customize" and then select the
SpamBayes toolbar and click "Delete". This will remove the
(non-functioning) SpamBayes toolbar. If you restart Outlook SpamBayes will
recreate the toolbar for you, and all should be well again.
