You can probably ignore the graylist directory errors -- they are most 
likely because those graylist entries were created by spamdyke version 
3.1.x, which used a different directory structure than 4.0.x.  If you 
want to clear the errors immediately, just delete everything in your 
graylist folder and let spamdyke recreate the structure as needed.

Beyond that, you haven't provided any additional information since your 
very first post.  I see your "log-level" option is set to "debug", so 
spamdyke should be logging statements to show which filters are matching 
the allowed connections.  Those log entries should tell you exactly why 
they're being allowed.
