Tom,

We use a floating div-layer menu.  If the user is logged in and belongs to
an appropriate group the menu is displayed.  It's default state is collapsed
and appears as a little red icon in the top right... when they click it
extends down with a variety of functions - one of which is to toggle to
designmode.

Other options have included (depending on the project):
webtop
flush caches
edit page
logout
etc

Very easy to use and can be configured to offer a variety of admin options
to appropriately privelged users.
