Hmm, how do you all feel about that warning message?  I think
there are cases where it distracts more than anything else,
but it really *is* easy to overlook the addition into the TOC.

I could offer an explicit way to suppress it per document, via
document metadata (i.e., putting ":orphan:" at the top of a
document would suppress the warning for it.)
