Hi,

when I mark an index entry as main entry like this:

.. index::
   ! single: Foo Bar

the entry is printed in bold.

But the entries are not sorted according to their position in the text
and not according to to their value – so the main entry is not always
the first entry giving the term itself.

How are index entries sorted? I would propose to sort them according
to their occurrence in the text.

Perhaps the main entry should be the first reference in the index
represented by the term itself.

What do you think?
