Which version of sphinx did you use? I made some changes to fix the problem which are already merged into the development version (commit f453d48beacc). There is one corner case that has not yet been integrated and I hope it will be merged soon. Could you check once more and report any further issues?
I saw that the amazon link offers a mobi/kindle version. Can you tell me how you created that version? Especially, how did you create the cover page and the table of contents entry? I have been experimenting with the creation of mobi files but still do not know how to best create them.
