Hi Kathy,
I don't know an answer to your first question (I'd like to hear the 
response as well), but for your second one, you can use the "only" markup, 
e.g.:

.. only:: html

   * :ref:`genindex` 
   * :ref:`search` 

so that these are skipped in the PDF output.
