Add a new rst file to the project and run make html. when you look at the html, you will see that the hierarchy shown at the top of the screen is missing and is not the full hierarchy.
This can be fixed by using the make clean html command, but this command will generate all the html, causing some unnecessary work and affecting the accuracy of "html_last_updated_fmt".
