First, you should make sure that the needle is actually adjusting properly, by removing the piston assembly, turning the adjustment, and verifying that the needle actually moves in and out. You might need to check that the needle and adjuster have not gotten forced upwards into the piston. Then check that the side of the needle and the jet (pressed into the carb body) are not badly worn. And of course make sure that the diaphragm is not damaged. Also make sure that a wrong or improperly installed gasket is not blocking the vent passages between the air filter and the carb, and the piston housing and carb body (assuming there is a gasket between them- I forget).
