I just left a store which was missing some outer rings part of a two piece semi 
recess escutcheon assembly. 
I examined the heads. They are Central heads installed with Viking escutcheons, 
installed throughout this large retail store! 
I think this practice is very wide spread. 
The AHJ's need to step up to the plate and look for this when systems are 
signed off on.
