If the state or locality has adopted NFPA 25 into the Fire Code or 
Building Code, the official could cite the property owner for non 
compliance.
I have seen similar situations where people actually commit fraud, in 
one case writing down main drain results when the pipe was covered with 
concrete!
It is useless to report it in most cases, but in a few cases the AHJ 
will actually do something.
The more I see, I wonder if we should just do away with NFPA 25 and let 
people do whatever they want because without enforcement it is 
impossible to have adequate compliance.

Was recently in a public building in my home town and saw a 2" drain 
piped
to a floor drain.  Wondering how this installer did a drain test for 
the
acceptance of the building - I asked the Building Inspector to see the 
Test
Papers supplied by the contractor.  The company had made their own
certificates - which did not include a main drain test under the test
section.  They simply never did the test so they omitted it from their
certificates.   And our Building Inspector never knew the difference.
Seems to be less than an honorable way to do business to me.
Anyone that makes their own forms should at least be required to 
include all
the necessary information on the forms.
