I checked with a few others on the NFPA 11 committee who regularly do 
inspections and we all agreed this form is suitable for Medium and High 
Expansion foam, as well. It should be relabeled as such. NFPA 11 will be open 
for comments in January of 2018 so I will submit the change then.
