The issue seems to have something to do with files in a project.

Assume a file tree structure as follows:
my_project 
    /*sparse_outline*.py
    /*sparse_outline*_2.py
misc_scripts
    /*sparse_outline*.py

For brevity, let *sparse_outline*.py  be the name of one of the files that 
exhibits the issue, and let's say it's a part of a Spyder project in the 
folder my_project. 

After copying *sparse_outline*.py to a folder that is not part of any 
project, I open the copied file in Spyder 4.2.5 while my_project is opened 
as a project.  The Outline pane behaves as expected, showing function, 
class, and method definitions.
