I like option 2 the best; it looks modern and simple. However, I don't think the menus are necessary; the toolbar is fine without them. I decided to try turning the page into a pure CSS layout with no tables. The result is available at http://www.cs.hmc.edu/~jdarpini/sqlite.html I tested it in IE6; 3 hopefully unobtrusive hacks were needed for it to render decently.
