I am very far from being very knowledgeable about custom-convert.conf
files but this seems to work on my system:

I used different equalizer settings so that I could audibly tell whether
it was working or not. You would need to change them back to your
preferred settings.
The changes I made are:
Removed the -D (no dither) after [sox]: not needed.
Removed the "-e signed": not needed for flac output. Also, curiously,
the Sox documentation on the Sox website doesn't include "signed" as an
option for "-e" (encoding). It has, amongst other options,
"signed-integer" and "unsigned-integer" but not "signed".
I have deleted the following sections as they are invalid. The third
line in each section should correspond to the Source and Destination
filetypes given in the first line of the section and that is not the
case.

Check whether my suggestion works in your system then, of you want to
use different source filetypes say what those are and I'll see what I
can do.
