Thanks for all the inputs guys. I found the issue following Django's suggestion. My "working directory" for tagging etc. is on my PC. My "music library" directory is on the NAS. In order to debug, I had - changed the tags and filename of the third item in the playlist on the PC - recreated the playlist - copied the new playlist over to the NAS - but forgot to copy the changed music file over Obviously LMS when reading the playlist could not find the referenced file, because the changed version was not present on the NAS. When I tried to create a playlist within LMS, I noticed the filename on the NAS was strange.
