If a package is not running after installation, often this is due to
failing module version check and building a package is a 2 minute job.
But in this case I should have asked another question first ...

Did you already run 0011 under DSM6.1? If so please check if the Trust
Level in Package Center Settings is still set to 'all pubishers'.
Sometimes this setting is changes during a DSM update.
