This can happen if 1) there are two instances of LMS running at the same time (unlikely, but still...) 2) LMS wasn't shut down properly
First, make sure 1) isn't the case Then, shut down LMS and check if there are any *.db-shm or *.db-wal files in the cache directory (e.g. library.db-shm and library.db-wal). If yes, delete them. Restart LMS, check the log again.
