Michael, Somehow I fixed it... I wiped my install over the weekend and started fresh from a clean RPi install and all my playlists seem to appear for both iTunes and Spotify.
I also figured out the >200 Spotify playlists was due to a bunch of playlists that are nested in folders... Saw there are some settings around hierarchical playlists, but given I rarely play those playlist... I'll just leave as is for now and play via Spotify Connect or search for them if necessary.
