Me too. I have seen plugins disappear (seriously) from the plugin page. Try restarting LMS then refreshing your browser. I think it has happened when a plugin is selected, then "apply", and "restart later" is selected, or, maybe if the plugin doesn't require a restart. It seems like after "apply" it disappears, but doesn't ever show up in active until LMS is using it/has restarted.
