I do hope that your investigation into the cache expiration strategy at mySB servers will get the issue out of the way.
Ok, found the issue: it's not cache related. But a considerable number of web hosts did not update correctly. They're still running the old implementation. Now whenever the load-balancer decided to move you to one of those hosts, you would get the old style menus.
