It was written internally at MARA Systems for their eMARA product and
was based on Squid-2.5, then ported on devel.squid-cache.org to Squid-2
HEAD and finally merged into Squid-3.

Indeed.

The Squid-2.5 features not merged into Squid-3 are

  - Location header rewrites

  - http_access2  (as http_access but after redirectors)

The reason is mainly due to overlapping changes in the area of request
processing where these hooks into the source making merging of these
features requiring a little more work. Probably easier to write again
from scratch within Squid-3 as each feature is quite trivial in nature.
