[PATCH] Make OPTIONS requests respect icap_persistent_connections

Hello,

The attached patch appends "Connection: close" to OPTIONS requests if
icap_persistent_connections is set to 'off'. This is inline with the
current behaviour for REQMOD and RESPMOD requests.
