Have you by any chance changed visible_hostname in squid.conf, and then
run a soft reconfigure (squid -k reconfigure)? This is known to break
icons.

My recommendation when changing squid.conf is to always make a complete
restart of Squid, unless you are 100% sure that reconfigure works for
the changes you have made.
