Squid configuration

I would like to know which documents are fetched directly from the
cache and sent to te client, and which ones are reloaded from the
original server and when . Squid seems to reload the files into the
cache more frequent than neccessary.This seems to me the only reason
that Squid is a bit slower than expected.
