When using non-icp peers Squid sends hierarchical requests to alive
parents. non-hierarchical requests are sent directly.

Be warned that trying to verify this with a web browser can be very
confusing as most testing attempts usually result in sending
non-hierarchical requests (Pragma: no-cache, caused by pressing the
reload button, or in some browser versions selecting a bookmark or even
typing a URL manually).
