I'm having an unusual problem with a squid setup that worked fine a few days
ago. A bunch of other stuff has been upgraded (including the kernel) since
then, maybe it's related?

Anyway, the problem is that squid (a vanilla config) periodically quits
downloading a file. In access.log, the "elapsed" field for the connection
goes impossibly high. Example below. You'll notice some SWAPFAIL's too -
they're pretty rare, but what causes them?

I had this problem with 2.2-dev3, and tried reverting to 2.1.2-4, but that
didn't fix anything. Overall, it's pretty confusing.
