Not if you are using glibc.

Does Squid continue processing requests while using 98% of the CPU, or
is it stuck doing something and then recovering?

You may be able to identify what Squid is doing by running "squid -k
debug" while it is using the CPU.. (run "squid -k debug" again to return
logging to normal levels).
