[squid-users] how median_svc_time calculated?

I am using SQUID 2.5 stable1
i want to know:
1) how median_svc_time is calculated for HTTP Requests........?
2) how Near Hits for HTTP Requests are calculated...?
3) how Not-Modified Replies are calculated........?
