On what format?

The NCSA helper expects a password file on the format

username:crypthashedpassword

The NCSA helper in Squid-3 also supports MD5hashed passwords (both 
Linux PAM and Apache style).

This is maintly an administrative choice.

Some people prefer to have the proxy use another user database 
available in their network such as a LDAP server, NT Domain, Novell 
NDS or whatever user directory they use.

If you are running the RedHat Squid RPM then the NCSA helper should 
already be installed in /usr/libexec/squid/.
