Well, I haven't tried using the auth_param entries so I'm not sure what that's about, but on my system when you use proxy_auth you also have to use authenticate_program check here:

Have you tried squid -k parse?
You may wish to change your http_access line to:
http_access allow users
Since the all part will always be true.
I would also put this right afterwards:
http_access deny all
