Are you using multiple deny_info's in your squid.conf file, because that can be the problem with Squid 2.5 stable 3.
To solve that problem, install the patch which you can download from the following location:
http://www.squid-cache.org/Versions/v2/2.5/bugs/#squid-2.5.STABLE3- deny_info
