I've started to isolate it. I'm using dansguardian so I don't know if it is there or in squid, I can't reproduce it with only dansguardian, and I can't reproduce it with just squid, but it happens every time with both.
I've turned debug_options ALL,9 on and am going line by line when it happens to something and am coming up with nothing right now (lots of logs to look through though), and i'm also not sure if this is the cause of the broken pictures either... I have a feeling I know what it may be, but I can't get at it until next week (poor me, got to do on-site support this weekend) to test and generate a fix. What I'd like you to do, is to give me any log messages you get in cache.log, store.log and access.log around the same time as the fault occurs in the bug.
