Each swap file has a header of meta data: URL, MD5, part of StoreEntry. 
That error means that meta data was missing or corrupted.

debug storeSwapMetaUnpack() to track the error. How long's been your squid 
running? Where you get Squid, ported application or the source?

i'm use openbsd 3.3, transparent proxy, diskd (with re-compile kernel
before), malloc.
