Looks like you are using the old SMB based ntlm_auth helper shipped with Squid. This helper is fundamentally broken and will always give errors from time to time. You may be able to limit the amount of errors by specifying very many login servers, but still there will be problems.

You should be using the Samba-3 ntlm_auth helper. This is proven and stable technology.

Yes, i switched to that one, and it looks good so far. Some users are still reporting (much lesser) auth popups, i will look into that now... maybe some br0ken IE or the user simply isnt allowed to use the internet...
