I disabled the challenge reuses and I still got it. When I get a popup there is 
nothing special in cache.log

This is when I restart squid with squid -k reconfigure:

I was wondering about the next entry a little while ago but I red that it was normal?! 
I have a lot of lines like that in cache.log

  fgets() failed! dying..... errno=0 (Success)
