On any FATAL errors the following procedure applies:
0. Read http://www.squid-cache.org/Doc/FAQ/FAQ-11.html#ss11.19
1. Get a stack trace of the error
2. Upgrade to lastest release, if not running this already
3a. If the problem gone away, be happy.
3b. Get a new stack trace of the error.
4. Look at the "known bugs" page to see if anything matches your problem
5. If not, file a bug report including the stack trace.
