ip_user_check is not an authentication mechanism, only an authorization mechanism restricting the authenticated users to their allowed IP addresses.
ip_user_check relies on authentication already being configured using your authentication schemes and mechanism of choice. Which to select depends very much on what user database you want to connect Squid to for the authentication.
