It is shipped as part of the distribution, installed when you install squid_ldap_auth.
man -M /usr/local/squid/man squid_ldap_auth
or
cd helpers/basic_auth/LDAP nroff -man squid_ldap_auth.8 | less
Note: the group helper is squid_ldap_group, in helpers/external_acl/ldap_group/
