Hello again I managed to get ntlm_auth working! Yay! Someone here pointed me towards disabling shadow passwords and straight away it started working correctly. The next thing on my to-do list is to trap expired passwords...
Presumably, since ntlm_auth should give (according to my little book) an NA response and a reason string to either an expired or locked account then squid is already handling this by logging a 407 error in access.log and generating another authentication request. I assume that a redirector of some description is needed to catch this; I am not expecting to be able to generate a password reset page but I'd at least like to be able to send the users to a page saying that they aren't being let in because... Can anybody throw any enlightenment my way?
