Sounds like you have set up a use quota on your bluecoat, or perhaps a limit in how many users may access the bluecoat from the same client IP (Squid).
Question: Is there a difference squid handles "login=PASS" different from "login=userxy:topsecret"?
The only difference is which account & password is used.
