In squid.conf file the entry is
auth_param basic program /usr/lib64/squid/squid_ldap_auth -b
"o=cd,dc=connectserver" -u cn -D "cn=Manager,dc=connectserver" -w
"[EMAIL PROTECTED]" -f cn=%s -v 3 -h 192.168.1.201

Which LDAP authentication helper are you using?
squid_ldap_auth from the distribution, or something else?
