We do appear to be using mod_deflate on our mediapool. Is it better to
exclude the file from this altogether?

Maybe you have a broken web server doing content encoding without
updating ETag? (I.e. Apache mod_deflate)
See the broken_vary_encoding squid.conf directive for a workaround.
