I tried Version 3.0.PRE6, this is just hell for ICQ.
In next five hours I'll try to roll-back to 2.6.STABLE14, but I'm not
sure this would help.
have you tried using the official ICQ client ??? i'm sure rolling
back to any other squid version wouldnt change anything, because this is
not a squid problem. This seems to be a client problem (Miranda in your
case), which seems to dont handle NTLM authentication requests from the
proxy.
