Is cachemgr.conf in the proper location? Either same directory as cachemgr.cgi (or to be exact the current working directory when cachemgr.cgi runs.. usually the same directory but depends on web server setup), or if not there then <prefix>/etc/cachemgr.conf If you are unsure about the <prefix> location then strings cachemgr.cgi | grep cachemgr.conf should tell you.
