My test also worked, using Squid 3.1.10. My best guess is "Connection-Pinning", a feature that does not exist in 2.5 or 3.0 branches. Please attempt to upgrade to either 2.7 or 3.1 branches, and see if that works.

When surfing out of squid, auth popup opens well. I tested out through a lot of different machines running different versions of squid, same issue.
